The Toe and Flow Model for Multidisciplinary Care of the Diabetic and Senior population

SPEAKER:  Dr. Brian Lepow
Chief of Podiatry, Director of Clinical Operations

Dr. Brian Lepow is a native Houstonian. He graduated from Westbury High School and attended the University of Arizona in Tucson, Arizona. He earned his medical degree from Barry University School of Podiatric Medicine in Miami, Florida. Dr. Lepow completed his surgical residency training at The Mount Sinai Medical Center in New York, where he served as chief resident in Podiatric Medicine and Surgery.


Following completion of his residency program he served as Fellow in Diabetic Limb Salvage and Reconstructive Surgery at the University of Arizona. In addition, Dr. Lepow held an academic appointment in the department of surgery at the University of Arizona College of Medicine.


It was during his fellowship that Dr. Lepow became acutely aware of the need for early diagnosis and formation of aggressive treatment plans in diabetics with lower extremity complications.


Dr. Lepow is a highly experienced Doctor of Podiatric Medicine (D.P.M.) with an extensive background in podiatric medicine and surgery specializing in Limb Preservation. Currently he serves as, Chief of Podiatry and Director of Clinical Operations at HOPE Vascular and Podiatry in Houston, Texas. In addition, Dr. Lepow has served as an Assistant Professor at the Michael E. DeBakey Department of Surgery, Division of Vascular Surgery and Endovascular Therapy, demonstrating his commitment to medical education and clinical expertise. His diverse roles in academia, clinical practice, and leadership underscore his dedication to advancing podiatric patient care.


In his free time Dr. Lepow enjoys spending time with his wife and 3 sons as well as traveling, participating in outdoor activities and exploring the growing Houston community.

CHARITABLE OUTREACH

The Houston Area Continuity of Care Board has established a discretionary fund targeted to help local, non-profit organizations serving Houston’s seniors through an annual monetary gift. HACOC members are encouraged to share nominations of eligible, non-profit corporate organizations with the HACOC Board for consideration. The Member-At-Large - Discretionary Fund Board member compiles a list of qualified non-profit corporate organizations which is presented to the Board for a vote by November of each year. The selected non-profit organization will be notified and recognized at the end of the year event.

CHARITABLE OUTREACH

Second Servings’ mission is to alleviate hunger and reduce waste in Houston by rescuing perfectly edible surplus food, and delivering it directly to local nonprofits. As the only perishable food rescue organization in Houston, they reach over 250,000 people annually, delivering high-quality, fresh food to over 150 Houston-area shelters, soup kitchens, low-income housing and other nonprofits from our generous donors, such as distributors, retailers, hotels, sports venues, etc. This perfectly edible food helps nourish abused women and children, at-risk youth, veterans, adults in recovery, disabled homeless seniors, low-income single adults and families and many others. Delivering the food at no cost allows their nonprofit partners to save a significantl on food budgets, enabling them to serve more people, provide additional services and pay overhead costs.

MISSION STATEMENT:

The purpose of the organization is to encourage communication among healthcare professionals and para professionals dedicated to the continuity of care for senior adults. Houston Area Continuity of Care provides member benefits through networking, education, and open communication – allowing members to remain up-to-date on the evolving healthcare industry.
